It isn't hard to figure out what happened, though; ever since that day a large part of her soul has been missing, as if it was ripped out. The once-bleeding wound scarred over and mostly healed since, but in the upper planes, it's easy to see that something is wrong there. He can't put his finger on exactly what until a shadowy creature crawls out of the wound.
